Scheduling & Cancellation

At rēVive Medical Aesthetics, appointment times are reserved specifically for you. To ensure availability and fairness for all patients, the following policies apply.

Appointment Cancellations & Rescheduling

• A minimum of 48 hours’ notice is required to cancel or reschedule any appointment.

• Cancellations, rescheduling requests, or failure to arrive for an appointment with less than 48 hours’ notice will result in applicable fees.

Deposits & Consultation Fees

• A $100 deposit is required for all injectable appointments and will be applied toward your final treatment balance.

• Deposits are refundable with at least 48 hours’ notice, unless the appointment is rescheduled.

• Cancellations or reschedules made within 48 hours will result in forfeiture of the deposit.

• All new patients are required to pay a nonrefundable $50 consultation fee, which may be applied toward treatment if services are performed.

Multiple Services & Appointment Modifications

• Appointments scheduled for multiple services reserve both provider time and product.

• Changes to scheduled services made at the time of the appointment are subject to a modification fee in the amount of your scheduled services.

Card on File Authorization

• A valid credit or debit card is required on file for all patients.

• By scheduling an appointment, you authorize rēVive Medical Aesthetics to charge the card on file for applicable cancellation, rescheduling, no-show, deposit forfeiture, or service modification fees in accordance with this policy.

Policy Acknowledgment

• Scheduling an appointment constitutes acknowledgment and acceptance of these policies.
